Secondary and Tertiary Rhinoplasti

The term secondary rhinoplasty is used to describe all secondary surgical interventions for the previously operated nose. To put it more clearly, it is the name given to the surgery to be performed on patients who are not satisfied with the surgery performed by another physician.

RESULT, WHY IS A SEPARATE DEFINITION NEEDED FOR THE SECOND SURGERY?

Because secondary interventions to the nose require more experience and some difficulties are encountered.

Adhesions due to the previous surgery occur in the surgical area, making it difficult to reveal the structures to be corrected.
Since the nose skin was opened before, the skin became thinner. It can show millimetric errors in the underlying bone or cartilage.

In the previous attempt, too much of the nasal bone or cartilage may have been removed.
A hole (perforation) may have formed in the midline cartilage in the nose due to the mucous membrane (intranasal skin) and cartilage damage that occurred in the previous surgery.
Since the previous operation was performed by another physician, it is not known exactly what was done to the patient in the first operation.
The operation time is longer.

In secondary rhinoplasty surgeries, it may be necessary to bring cartilage or fascia (muscle membrane) from another part of the body in order to provide the old support to the nose.

In my personal experience, almost 90% of secondary rhinoplasty operations may require this type of graft. Usually, there is collapse in certain parts of the nose because the cartilage or bone in the nose was removed in the previous surgery. These collapses can occur not only after uncontrolled aesthetic surgeries, but also in cases where this cartilage is taken too much in the curvature of the cartilage in the midline of the nose.

In these surgeries, I first tell my patients that ear or rib cartilage can be removed, and I will probably decide which one to take or not, depending on the situation in the surgery.

In secondary rhinoplasty surgeries, plaster splint and then bandage application procedures can be applied. If a serious intervention is made into the nose, it may be necessary to place silicone tampons. Apart from this situation, tampon application is not required frequently.
